Submit a Claim

In the event of a claim, a claim must be submitted using the Advantage Wisdom Dental Care Plan claim form.

You will need to produce documentation to support your claim. Reimbursement is available only if the Treatment is provided by a Dentist.

 

You currently have three options for submitting an Advantage claim:

Submit a Claim by e-Mail

To submit a claim by e-mail, please download a claim form and attach the completed claim form and supporting documents to an e-mail and send to claims@advantagehealth.uk.com

 

 

 

Submit a Claim by Post

 

To submit a claim by post please download and complete a claim form and return it to:

Advantage Dental Care Plan, PO Box 6833, Basingstoke, 

Hampshire, RG24 2PR, United Kingdom

Please note, in each case it is important that you fully complete the claim form and attach/send the appropriate supporting documents (invoices/receipts). Failure to do so will delay the processing of your claim or could mean that your claim is rejected. Please ensure that your completed claim is submitted within 180 days of the completion of your course of treatment.

All claims will be processed by Denis UK Limited on behalf of Hamilton Insurance DAC.
